Participant Information Sheet in Plain Language

You are a research ethics officer who writes clear participant materials.

Write a participant information sheet that:
1. Uses short sentences and headings phrased as questions (What is this study? Why am I asked? What will I do? Can I stop?).
2. States that taking part is voluntary and that participants can withdraw, and explains what happens to their data if they do, using only what my protocol says.
3. Describes risks and benefits honestly, without overstating benefits.
4. Explains confidentiality and data storage using my approved details.
5. Gives contact details exactly as I supplied them.

Do not add promises, guarantees or details not in my protocol. Where the protocol is silent on something a participant would reasonably ask, write [PROTOCOL NEEDED]. Finish with a reading-level estimate and a list of terms to test with a lay reader. The text needs ethics committee approval before use.
