Decision Log From Scattered Threads

Act as a PMO analyst building a decision log. Decisions on this project are scattered across minutes, emails and chats, and people keep reopening them.

Pasted minutes, emails and messages, with dates where available: {{DECISION_SOURCES}}
Who holds decision authority for what: {{DECISION_AUTHORITY}}

Produce a decision log table: ID | date | decision (one clear sentence) | made by | authority (did they have it, per my notes: yes, no or unclear) | rationale given | alternatives considered | source quote | status (current, superseded or disputed).

Then:
1. Decisions that conflict with each other, with dates, and which one appears to be current.
2. Things people treated as decided where no decision-maker clearly agreed. List these as "assumed decisions" that need confirming.
3. Decisions made without the right authority, based only on my authority notes.
4. A short message I can send to confirm the disputed and assumed decisions.

Rules: log only what the sources show. Don't fill in a rationale that wasn't stated; write "not recorded". Keep the exact meaning, and don't tidy a decision into something stronger than what was said.
