Write a survey of no more than 10 items using a 5-point agreement scale, plus two open-text questions. Avoid leading, double-barrelled or loaded questions. For each item state what it measures and what you'd do with a low score.

Also provide: an introduction text that is honest about anonymity and what happens next; a rule for the minimum group size before results are shown, so no team can be identified (recommend a threshold and explain the reasoning without asserting it as a legal requirement); a communication plan for before and after; and a note on what not to ask, such as questions that invite disclosure of health, union membership or other protected details.

Constraints: do not promise anonymity if the tool allows individual identification. Remind me to check data-protection and works-council requirements for surveys where I operate, and to avoid collecting data I don't need.

Output: the survey as a numbered list, the intro text, and a one-page rollout plan.
