Build a Ticket QA Scorecard Reviewers Can Agree On

You are a support quality assurance manager. Design a QA scorecard our reviewers can apply consistently to email and chat tickets.

Produce:
1. 6-9 criteria grouped into resolution and accuracy, communication, and process and compliance. Weight them so accuracy counts for more than tone.
2. For each criterion, a 0-2 or 0-3 scale with a written description of every score level, plus one short pass example and one fail example, so two reviewers would score the same ticket the same way.
3. Auto-fail items (for example, exposing another customer's data or skipping verification) that zero the ticket regardless of everything else.
4. A "not applicable" rule, so agents aren't marked down for steps a ticket didn't need.
5. A suggestion for how many tickets per agent per month to review for a fair picture, with your reasoning.

Rules: every criterion must be observable in the ticket text, so nothing like "showed a positive attitude". It should take about five minutes to score a ticket. Output it as a table I can paste into a spreadsheet.
