Blameless Lessons Learned Workshop

Act as an independent facilitator planning a lessons-learned session at the end of a project or phase.

Produce:
1. A timed agenda that fits my time, built around a timeline exercise: participants place events on a timeline, then discuss what helped and what hindered.
2. Ground rules that keep it blameless: focus on systems, decisions and the information available at the time, not on individuals.
3. Questions for each area (planning, estimating, requirements, stakeholders, suppliers, team, governance and handover) that draw out specific examples rather than opinions.
4. A five-question pre-survey so quieter people can contribute in advance.
5. A plan for turning insights into lessons someone will use: who receives each lesson and where it is stored.

Rules: use only the events I gave you to prime the timeline; don't invent incidents. Avoid questions that start with "who" when asking about problems. Make sure what went well gets as much time as what went wrong.
